## 示例文字 / Sample wording

本人／本公司 `[法定名稱 / legal business name]` 授權 `[指定第三方的法定／交易名稱 / named third party’s legal/trading name]`，只可就 `[列明供應地址／供應點 / named supply address or supply point]` 向 `[列明供應商 / named supplier]` 索取現有供應資料。

I/we `[legal business name]` authorise `[named third party’s legal/trading name]` only to request current supply information for `[named supply address/supply point]` from `[named supplier]`.

此授權不包括代本人／本公司簽訂、續約、修改、取消或轉讓能源合約；不包括委任其他代理人；亦不包括在已列明目的以外使用或分享資料。

This authority does not include entering into, renewing, varying, terminating or transferring an energy contract; appointing another agent; or using or sharing data beyond the stated purpose.

本授權由 `[開始日期 / start date]` 至 `[到期日期 / expiry date]` 有效，可按 `[書面撤銷方法 / written revocation method]` 撤銷。

This authority applies from `[start date]` to `[expiry date]` and may be revoked by `[written revocation method]`.
